“Walking Hand-in-Hand'' Cancer Family Support Project (Tai Po Centre)
目標 Objectives
改善患者及其照顧者的生活質素以及減輕他們在抗癌路上的壓力; 確保患者由醫院過渡至社區的照顧接軌;
建設一個容易接觸的平台，讓不同階段的癌症患者及其照顧者及時地獲取所需資訊 及支援;以及
培育跨專業的協作模式及建立社會網絡支援癌症患者及其照顧者的心理社交需要。
To improve the quality of life of patients and their caregivers, as well as to alleviate the pressure they face fighting cancer;
To establish an easily platform on which patients of different stages and their caregivers can obtain the information and support required in a timely manner; and
To nurture a multi-disciplinary collaboration model and to establish a community network to support the psychosocial needs of cancer patients and their caregivers.
