可予改善的服務層面,並讓我們能夠宣揚
病人安全文化,加強與前線員工的溝通和
支援。
本院作為大埔區的主要醫護服務機構,我
們必須持續檢討本身的護理和服務模式,
以滿足區內未來的醫療服務需要。本院根
據聯網服務發展的整體方向,並在海外顧
問的協助和聯網員工高度互動及廣泛參與
下,與醫院管理局(「醫管局」)攜手訂立
臨床服務計劃。該計劃清晰地闡明主要的
服務改善、護理模式和各臨床專科的工作
建議,構成了引領本院發展臨床服務的核
心參考資源。綜合本院面對的挑戰,以及
我們的優勢和潛力,參與規劃的成員均同
意本院將肩負地區急症全科醫院的角色,
逐步發展成為聯網指定的日間及短暫留院
中心,並重新建立本院急症手術服務。此
外,本院與大埔醫院將合併臨床服務與管
理,務求改善兩院之間的護理延續性及病
人流。
為推動更佳的營養護理,及早並有效地介
入處理營養不良個案,營養治療部提升營
養評估標準化護理並適時介入,妥善照顧
中風病人和營養不良高風險病人,以及為
病人和他們的照護者提供相關支援,藉以
加強出院後延續護理。員工是推動本院護
理質素的關鍵,因而為二級病人助理引入
能力為本的培訓計劃,讓員工適切地掌握

strengthening communication and support to frontline staff.
As the key healthcare provider in Tai Po district, it is imperative
for AHNH to continually review its care and service models with
a view to enabling the latter to meet the healthcare needs of the
community in the years to come. Based on the overall direction
of delineating the service development for the cluster, AHNH and
the Hospital Authority (HA) jointly developed the Clinical Services
Plan, which has been commenced with assistance from overseas
consultant and developed through highly interactive and broad
engagement process with cluster staff. The plan articulates the
key service improvements, models of care and recommendations
by clinical specialties and forms a core reference to guide the
development of the clinical services at AHNH. Taking into the account
of the challenges faced by AHNH and building on its strength and
potential, there is a consensus that the hospital will function as
acute district general hospital, develop into the cluster’s designated
ambulatory care and short stay centre and re-establish its emergency
surgical services. In addition, AHNH and TPH will amalgamate
clinically and managerially, so as to improve the continuum of care
and patient flow between the two neighbouring hospitals.
In order to promote better nutrition care enabling early and effective
interventions of malnutrition, the Department of Dietetics has
enhanced standardized care practice in nutrition assessment and
intervention for stroke patients and high malnutrition risk patients, as
well as peri- and post-discharge dietetic support for patients and their
community carers to enhance continuity of care. As staff competency
essentially contributes to quality of care delivered, the department
